Case Management Services

Telephonic Case Management
Telephonic case management services are generally utilized in an early intervention capacity. However, telephonic services may be utilized during any phase of the case management process when deemed appropriate for cost containment efforts.

Task Assignment
Task assignments are generated for the purpose of accomplishing specific activities with the most limited level of involvement. They are frequently used in conjunction with a telephonic case management component or as a stand-alone service.

Tennessee Face-to-Face
Flat rated service created to assist with the Tennessee Case Management Rules for cases requiring an initial face-to-face meeting with a field case manager, effective January 1, 2017. Case management is not mandatory, but once a telephonic or field case manager is assigned, the face-to-face must occur within 14 days. (Case Management Rules 0800-02-07-.03, Case Management Threshold)

Field Case Management
Field case management is typically utilized when the diagnosis has a projected disability duration of greater than 90 days or factors prolonging claims resolution. It provides a comprehensive exchange of information and coordination of case management activities between the client, employer, medical provider(s), and payor. The case manager functions as a liaison and facilitator toward the goal of maximum medical improvement and successful return to work in a modified or full duty position.

Catastrophic Case Management
Illnesses and injuries that are serious, life changing, and potentially life-threatening require immediate intervention. Without proper management, recovery may be delayed and cost becomes excessive. Eckman/Freeman trains our CATA nurses to recognize and urgently respond to the immediate needs of the client, assuring the most appropriate care is received while providing family support and guidance.
